Understanding the Latest Regulatory Developments in Peptide Therapy

During its July meeting, the Pharmacy Compounding Advisory Committee reviewed seven peptide-related substances to determine whether they should be considered for inclusion on the FDA’s Section 503A Bulk Drug Substances List.
After two days of scientific presentations, expert testimony, and committee discussion, the advisory panel recommended that six peptides move forward for consideration:
- BPC-157
- KPV
- TB-500
- MOTS-c
- Epitalon
- Semax
The committee did not recommend emideltide (DSIP) for inclusion.
While this represents an important milestone, it’s equally important to understand what these recommendations do—and do not—mean.
What Is the 503A Bulk Drug Substances List?
Section 503A of the Federal Food, Drug, and Cosmetic Act establishes the conditions under which licensed pharmacists and physicians may prepare patient-specific compounded medications.
For a bulk drug substance to be used in traditional pharmacy compounding, it generally must satisfy specific regulatory requirements, including appearing on the FDA’s 503A Bulk Drug Substances List when appropriate.
The Pharmacy Compounding Advisory Committee provides recommendations to the FDA regarding which substances should be included.
However…
The committee’s vote is advisory—not final.
The FDA must still review the recommendations and complete its own regulatory process before any changes become official.
This Is Not the Same as FDA Approval
One of the biggest misconceptions surrounding the July meeting is the belief that these peptides are now “FDA approved.”
They are not.
FDA approval requires extensive clinical research demonstrating that a drug is safe, effective, consistently manufactured, and appropriate for a specific medical use.
Compounded medications follow a different regulatory pathway.
A favorable recommendation for inclusion on the 503A list simply creates a possible pathway for qualified compounding pharmacies to prepare patient-specific prescriptions if all federal and state requirements are met.
Patients should therefore be cautious of advertisements claiming these peptides have been “approved by the FDA.”
That would be an inaccurate interpretation of the committee’s actions.
The Six Peptides Recommended for Further Consideration
Each peptide reviewed during the meeting has generated interest for different potential clinical applications.
BPC-157
BPC-157 has attracted attention for its potential role in:
